Only the individual or a parent may request the record. State issued photo id is required. Please enlarge and lighten before faxing If none is available, send copies of utility bills, income tax records, medical bills, ect ...to prove your identity.

Anyone born in New York City must provide a valid state issued id. Certificates are only issued to the certificate holder or a parent. A credit card must be in the parent or certificate holders name. If this is not possible, a money order can be sent by us and charged to a credit card. This is a slower process as credit card orders are done much faster.
